Application for admission has to be made on a plain paper.
There is no printed prescribed form for admission.
You have to furnish details in your bio-data.
All emphasis is on the tran-script and your performance.

You have to enclose your views on any one subject,
on a separate paper in the style of an essay.

You have to attach a separate sheet stating your extra-curricular activities.

Obtain recommendations of two of your teachers.
Get recommendation of one Counsellor to fill out your recommendation.
